Who’s in it.
30 telehealth and D2C health brands sold to US consumers, across six sub-verticals: personal care telehealth (Hims & Hers, Keeps, Curology, Midi Health, Flo Health), weight loss and GLP-1 (LifeMD, WeightWatchers, Ro, Noom), online therapy and mental health apps (Calm, Talkspace, Rula, BetterHelp, SonderMind), therapy networks and clinics (Headway, Grow Therapy, LifeStance), care access and testing (Function Health, Zocdoc, Quest Health, Everlywell, Prenuvo, GoodRx) and D2C health products (WHOOP, Zenni, AG1, Oura, Warby Parker, 1-800 Contacts, Thorne).
What we pulled.
Every brand’s Meta Ad Library catalog, Google ads from the Google Ads Transparency Center, the total number of Google ads per brand (all regions, all advertiser accounts), and every ad pixel BuiltWith has detected on each brand’s website.
How it’s scored.
Every score uses fixed thresholds, not a curve, so a 75 means the same thing for every brand and all 30 sit on one scale.

Key findings
The short version
01
The top of the table: Hims & Hers (89), Function Health (80), Calm (75), WHOOP (73) and LifeMD (73). The sector median is 58.
02
More channels didn’t buy better ads. 7 of the 11 brands running 3 or more paid platforms sit below median ad health. 11 of the 19 running only Meta and Google sit at or above it.
03
Leaders keep their Meta winners running. In the top 10, a median 2% of Meta ads die inside 14 days and 64.5% have run for 60 days or more. In the bottom 10 it’s 31.5% and 15%.
04
Leaders keep Google moving. The top 10 launched a median 10.5% of their Google ads in the last 30 days. The bottom 10 launched 1.5%.
05
The Meta pixel is leaving the website. All 30 brands run Meta ads and all 30 sites once fired the Meta pixel. Only 6 still do.
06
The top two run no live ad pixel at all. 10 of 30 sites carry none, including Hims & Hers and Function Health.
07
What’s new is small and recent. OpenAI’s ad pixel went onto 3 sites between May and September. SiriusXM audio tags went onto 2 sites in the first week of October.
01 · The 2x2
The telehealth and D2C health 2x2
The two best-run ad programs in this set show live ads on exactly two platforms: Meta and Google. Hims & Hers and Function Health don’t run a single other ad platform we can see, and they sit first and second.
We plotted every brand on two things. Across the bottom: how many paid ad platforms show live activity, counting the ad libraries plus every ad platform pixel that fired in the last 90 days. Up the side: ad health, the average of each brand’s Meta Health and Google Health scores. Both lines split at the sector median.
Channel breadth vs ad health
Fig. 1
Meta Ad Library, Google Ads Transparency Center, BuiltWith. Splits at sector medians: 2 platforms, 67.1 ad health. Dots at 2 platforms nudged apart to stay legible.
Most brands sit on the left of the chart. 19 of the 30 run only Meta and Google among the ad platforms we detected, and 11 of the 15 brands with above-median ad health are in that group, including Hims & Hers, Function Health and LifeMD (#1, #2 and #5 overall). WeightWatchers is the widest at 13 live ad platforms. The next widest, Curology, Noom and 1-800 Contacts, run 7.

Breadth and ad health move in opposite directions here (a correlation of -0.32). 7 of the 11 broad brands have below-median ad health. Focused operators is the largest group: 11 brands that keep Meta and Google healthy and stop there.
Hims & Hers is the clearest case. It runs between 10,000 and 20,000 Google ads and 6,026 ads in the Meta Ad Library, carries no live ad pixel on hims.com, and scores 95 on Meta Health and 100 on Google Health. 56% of its Google ads have run for 180 days or more, and it still launched 12% in the last 30 days.
02 · The leaderboard
The leaderboard
Overall score is the average of four parts: Meta Health, Google Health, format diversification and operating balance between the two platforms. Here’s the top 10.
| # | Brand | Sub-vertical | Overall | Meta Health | Google Health | Live platforms |
|---|---|---|---|---|---|---|
| 1 | Hims & Hers | Personal care telehealth | 89 | 95 | 100 | 2 |
| 2 | Function Health | Care access & testing | 80 | 100 | 84 | 2 |
| 3 | Calm | Online therapy & mental health apps | 75 | 78 | 80 | 3 |
| 4 | WHOOP | D2C health products | 73 | 89 | 72 | 3 |
| 5 | LifeMD | Weight loss & GLP-1 | 73 | 74 | 100 | 2 |
| 6 | Headway | Therapy networks & clinics | 71 | 73 | 89 | 2 |
| 7 | Talkspace | Online therapy & mental health apps | 69 | 86 | 62 | 2 |
| 8 | Zenni | D2C health products | 69 | 89 | 75 | 5 |
| 9 | Grow Therapy | Therapy networks & clinics | 66 | 78 | 72 | 2 |
| 10 | AG1 | D2C health products | 65 | 67 | 72 | 2 |
| Sector median (30) | 58 | 74 | 63 | 2 |
The main route to the top is narrow. 7 of the top 10 show live ads on Meta and Google only. Calm and WHOOP add one more platform each (Microsoft Ads and Reddit), and Zenni is the widest of the ten with 5. Three brands post a perfect score on one platform: Hims & Hers and LifeMD on Google, Function Health on Meta. Therapy and mental health takes four of the ten places (Calm, Headway, Talkspace, Grow Therapy).
03 · What leaders do
What the leaders do differently
The gap between the top and bottom of the table isn’t volume. It shows up in how long ads stay live on Meta and how often new ads go into Google.
Top 10 vs bottom 10
Fig. 2
Medians. Meta Ad Library, Google Ads Transparency Center.
On Meta, they keep their winners running
In the top 10, a median 2% of Meta ads die inside 14 days and 64.5% have run for 60 days or more. In the bottom 10 it’s 31.5% and 15%. 1 of the top 10 loses more than 20% of its Meta ads inside two weeks. 7 of the bottom 10 do.
The bottom 10 aren’t short of new ads. They launched a median 17% of their Meta ads in the last 30 days, against 4% for the top 10. Their ads just don’t last. The top-10 brand that tests hardest is LifeMD: 80% of its sampled Meta ads went live in the last 30 days.
On Google, they keep launching
Top-10 brands launched a median 10.5% of their Google ads in the last 30 days. The bottom 10 launched 1.5%, and 8 of them launched under 5% of their Google ads in the last month. 1 of the top 10 did. Over 90 days the gap holds: 32.5% against 5.5%.
Video adds to the gap. A median 15% of the top 10’s Google ads are video, against 5% for the bottom 10. Talkspace (70%), WHOOP (65%) and AG1 (62%) lean hardest.
Leaders also spread Google across formats. The median top-10 brand puts 70% of its Google ads in its biggest format. The bottom 10 put 92.5%, and 7 of them run 90% or more in one format. 2 of the top 10 do.
Google ads by format
Fig. 3
Google Ads Transparency Center, up to 500 ads per domain, October 2026. Shares rounded per format, so a few bars total 99% or 101%. Ranks 21–30 shown by sub-vertical.
The bottom 10’s Google programs are also old. A median 85% of their Google ads have run for 180 days or more, against 39.5% for the top 10. On Meta it’s the other way around: the bottom 10 lose a median 31.5% of their ads inside 14 days, the top 10 lose 2%. Leaders refresh Google and hold Meta steady.
The exception worth knowing
93
Meta Health
36
Google Health
Oura has the third-best Meta Health in the set (93) and a Google Health of 36. It launched 39% of its Google ads in the last 30 days, and 1% have run for 180 days or more. Refreshing Google is what the leaders do, but Oura shows it can be overdone: nothing stays live long enough to become a proven ad.
04 · Channels
Which channels they use
Every brand in the set runs Google ads and Meta ads. After that, most stacks stop. Here’s what’s live across the 30 brands’ websites:
| Channel | Brands live | Main platforms (brands) |
|---|---|---|
| Search and display | 20 | Google Ads (20), Microsoft Ads (8), Yahoo (1) |
| Social | 11 | Reddit (8), TikTok (7), Meta (6), Pinterest (4), Snapchat (2), X (2), LinkedIn (1) |
| AI answer surfaces | 3 | OpenAI (3) |
| Programmatic | 3 | AppLovin (3) |
| Audio | 2 | SiriusXM (2) |
| Connected TV | 1 | Roku (1) |
| Direct mail | 1 | PebblePost (1) |

Ad pixels: live vs went quiet
Fig. 5
BuiltWith, pulled 2026-10-12. Mobile measurement partners excluded. A quiet pixel means the site stopped reporting, not that the channel stopped.
The Meta gap. All 30 brands advertise on Meta and all 30 websites once fired the Meta pixel. Only 6 still do, and 24 have stopped. In the top 10, only Calm and LifeMD still fire it. Meta is most likely getting its signal from apps and server feeds, not the website, which also means the number Meta reports back is the main number most of these teams see.
Several leaders run no ad pixels at all. 10 of 30 websites carry no live ad pixel of any kind, including Hims & Hers, Function Health, Headway and AG1. All 10 still run ads on Meta and Google. Whatever measures those ads now sits somewhere a crawler can’t see.
05 · What’s changing
What’s emerging, what’s going stale, what’s changing
Rule used: a pixel is live if BuiltWith saw it in the last 90 days. It went quiet if it hasn’t been seen for 90+ days. “Added” uses the first date BuiltWith detected it.
Emerging
OpenAI’s ad pixel. 3 of 30 sites carry it and all three were added this year: Zenni (May 30), WeightWatchers (July 16) and Everlywell (September 8). None has dropped it. 23 of 30 sites carry some OpenAI technology, but only these 3 carry the ad pixel.
Audio. SiriusXM’s audio ad tag went onto 1-800 Contacts on October 3 and onto WeightWatchers on October 5.
A build-out in real time. WeightWatchers added OpenAI’s ad pixel on July 16, then AppLovin and SiriusXM on the same day, October 5. 13% of its Google ads went live in the last 30 days, against a sector median of 4.5%. It now runs 13 live ad platforms, the widest stack in the set.
Reddit. Live on 8 of 30 sites. Prenuvo added it on April 19.
Going stale
Google’s own tag, LinkedIn, X and TikTok. In the last 12 months, the Google Ads tag went quiet on 7 sites, LinkedIn on 6, X and TikTok on 5 each, and Microsoft Ads on 4.
The long tail of social. Over the full history, X has gone quiet on 21 of the 23 sites that ever ran it, LinkedIn on 17 of 18 and Yahoo on 16 of 17.
What nobody dropped: OpenAI. The newest channel is the only one with no exits.
What’s changing
Tags are going quiet while ads keep running. AG1’s Google, Microsoft Ads, Reddit, TikTok, X, AppLovin and Outbrain tags were all last seen on the same day, May 11, 2026. AG1 still has 3,000 to 4,000 Google ads and 8,326 ads in the Meta Ad Library, and launched 20% of its Google ads in the last 30 days. Hims & Hers’ Google tag was last seen July 2, 2026, and it runs 10,000 to 20,000 Google ads. Everlywell’s LinkedIn, Microsoft Ads, TikTok and X tags were all last seen July 5. None of these brands stopped advertising. Their measurement moved somewhere a crawler can’t see: server-side tagging, app SDKs, or both.
That’s a sensible move for brands that sell through apps and logged-in accounts. It also moves one more piece of the measurement stack out of the brand’s own view and into the platforms’ reporting. 7 of the 30 run a mobile measurement partner on their site (Branch on 3, AppsFlyer on 3, Adjust on 1), and 8 more ran one in the past. Only 3 run a web-based multi-touch tool, all of them Rockerbox: WeightWatchers, Grow Therapy and Everlywell. For most of this sector, the only read on what a channel drove is what that channel says it drove.
06 · Creative
Creative and messaging
We labeled the top 15 Meta ads for each brand (fewer where a brand has fewer) by what they lead with.
Leaders pick one message and commit. In the top 10, a brand’s main message takes a median 70% of its top ads, and 9 of the 10 put at least half their top ads behind one message. In the bottom 10 the median is 56.65% and 6 of 10 do.
Price is common, but it doesn’t separate the table. 8 brands lead with price or promo, and 4 of them are in the top 10: Zenni (80% of top ads), Grow Therapy (80%), Talkspace (73.3%) and AG1 (60%). The median share of price-led ads is 26.7% in the top 10 and 30% in the bottom 10.
Clinical brands lead with what they do. 5 brands lead with the product or service itself, and 3 of them are in the top 10: Function Health (73.3%), Headway (66.7%) and LifeMD (60%). For a lab panel, a therapist match or a prescription, the service is the pitch.
Use case leads at the top too. WHOOP puts 73.3% of its top ads on a use case or life moment, and Hims & Hers 53.3%. 7 brands lead with it in total.
Almost nobody sells trust. In a category built on prescriptions, lab results and therapy, 0 of 30 brands lead with trust, privacy or safety, and only 3 use it at all. Social proof is rare too: 10 brands use it and one, Flo Health, leads with it (60% of top ads).
07 · Sub-verticals
By sub-vertical
| Sub-vertical (medians) | Overall | Meta Health | Google Health | Live platforms | Google ads that are video | Google ads per domain | Meta library ads |
|---|---|---|---|---|---|---|---|
| Therapy networks & clinics | 66 | 73 | 73 | 2 | 1% | 2,500 | 260 |
| Online therapy & mental health apps | 65 | 76 | 63 | 2 | 11% | 1,500 | 179 |
| Personal care telehealth | 61 | 66 | 78 | 2 | 7% | 1,500 | 1,399 |
| D2C health products | 59 | 74 | 48 | 3 | 28% | 3,500 | 637 |
| Weight loss & GLP-1 | 55 | 81 | 64 | 4.5 | 11.5% | 2,500 | 1,228 |
| Care access & testing | 54 | 74 | 47 | 2 | 8.5% | 350 | 267 |
Therapy is the strongest corner of the set: therapy networks and clinics have the highest median (66) and online therapy is next (65), and together they place 4 brands in the top 10. Therapy networks also run the least video on Google of any group (1%). Personal care telehealth is the strongest on Google (78). Weight loss and GLP-1 has the healthiest Meta (81) and the widest stacks (a median 4.5 platforms). D2C health products is the most split group, with 3 brands in the top 10 and 3 in the bottom 10, and it runs the most Google video (28%) and the most Google ads (a median 3,500 per domain). Care access and testing has the weakest Google (47) and the smallest Google programs.
08 · What we’d do
What we’d do with this
The lesson from the 2x2 is that every channel is a position, and each one has to earn its size. In this sector the leaders hold two positions and run them well: 7 of the top 10 show live ads on Meta and Google only, and 7 of the 11 brands that went wider sit below median ad health.
Three things we’d act on if we ran growth at one of these brands:
1
Keep your Meta winners live and keep Google moving. If more than 20% of your Meta ads die inside two weeks, you’re testing without keeping winners: 1 of the top 10 does that and 7 of the bottom 10 do. If you launched under 5% of your Google ads in the last month, your Google program is coasting: 8 of the bottom 10 are there and 1 of the top 10 is.
2
Prove a new position before you size it. OpenAI’s ad pixel and SiriusXM audio are where this sector is adding: 3 brands added OpenAI’s pixel since May and 2 added SiriusXM in October. Each is a good candidate for a geo holdout: turn it off in matched regions for three to four weeks and measure the incremental lift, instead of scaling on the platform’s own number and defending it after the fact.
3
Know where your measurement went. 24 of the 30 sites stopped firing the Meta pixel and 10 run no live ad pixel at all, including the top two. Only 7 run a mobile measurement partner on their site and 3 run a web-based multi-touch tool. That’s fine as long as you have an independent read on what each dollar drives. Most of this sector doesn’t.

If you run growth at a telehealth and D2C health brand and want to see where you’d land on this chart, we’ll run your brand against the set.
Appendix A
Deeper findings
The detail behind the headline findings, plus the things that didn’t fit the main story but are worth knowing.
01
A partner runs a share of Rula’s Google ads. Rula’s own account runs 75% of the Google ads pointing at rula.com. ZEFR, a YouTube ad partner, runs 500 to 600. Reporting from the brand’s own ad account misses that share.
02
Two perfect Google Health scores. Hims & Hers and LifeMD both score 100 on Google Health. Both keep about half their Google ads running for 180 days or more (56% and 45%) and still launched new ones in the last 30 days (12% and 7%). That balance between proven ads and fresh ones is what the score rewards.
03
Function Health is the only perfect Meta Health score. 100 on Meta Health, 0% of its sampled Meta ads dying inside 14 days, and a Google tag that went quiet in January 2026 while 200 to 300 Google ads keep running. It’s #2 overall on two platforms.
04
The fastest Google launcher is in the bottom 10. One brand there averaged 37.17 new Google ads a week over the last 12 weeks. In the top 10, AG1 (25), Calm (23.25) and WHOOP (21.42) are the fastest. Speed alone doesn’t decide the table.
05
Calm’s Meta catalog dwarfs the set. Calm has 39,508 ads in the Meta Ad Library, almost five times the next brand, AG1 (8,326). 83% of its sampled Meta ads have run for 60 days or more.
06
Google video clusters in a few brands. Talkspace (70%), WHOOP (65%) and AG1 (62%) run the most video-heavy Google programs. Therapy networks run a median 1%. Video is one of the clearest open tests in the sector.
07
The oldest Google programs sit at the bottom. The three brands with the oldest Google ads, a median ad age of 1,569, 1,254 and 1,187 days, are all in the bottom 10. Ads that stay up for three years aren’t proven winners by default.
08
Fewer than a quarter run mobile measurement. 7 of 30 run a mobile measurement partner on their site, against 15 that have had one at some point. 6 of the top 10 run none: Function Health, WHOOP, LifeMD, Headway, Zenni and AG1. 3 of the 5 online therapy apps run one.
09
The leanest stack sits at the top. Hims & Hers shows ads on Meta and Google only, has no live ad pixel on its site, runs 10,000 to 20,000 Google ads and 6,026 ads in the Meta Ad Library, and ranks #1. Pixel counts alone understate brands like this, which is why the 2x2 counts ad library activity too.

Appendix C
Full inventory
Platform counts are live ad platform pixels only (seen in the last 90 days), so Google and Meta count only where their pixel fires on the site. That is why a count here can sit below the same brand in Appendix B, which also counts Google and Meta from their ad libraries. Each brand expands to its complete stack: every pixel with its first-detected date, live channels, attribution tools, OpenAI tags, 90-day additions, 12-month drop-offs, and the sell-side and identity tags excluded from the count.
Mobile measurement = runs a mobile measurement partner (AppsFlyer, Branch or Adjust) on its website. Adding platforms = added one or more new ad platform pixels in the last 90 days and now runs 2+ ad platforms. Both = both patterns.
